The company's financial health has shown signs of recovery. In 2025, UnitedHealth experienced a notable drop in operating earnings, from $32 billion to $19 billion, primarily due to an increase in its medical care ratio (the proportion of premiums spent on claims) to 88.9% from 85.5% in the previous year. However, recent data indicates a reversal of this trend, with the medical care ratio falling to 86.7% in the second quarter, surpassing analyst expectations of 88.5%. Management now projects a full-year ratio of 88%, with aspirations to achieve an even lower figure. This positive shift is also reflected in the second-quarter revenue, which reached $112 billion, exceeding the $110.9 billion forecast, and adjusted earnings per share of $6.38, significantly higher than the $4.90 consensus estimate.

Following these encouraging results, UnitedHealth has increased its full-year adjusted EPS guidance to a range of $19.50 to $20, up from its previous projection of $18.25. The company attributes this improvement to strategic restructuring, including the termination of unprofitable contracts, and the innovative application of artificial intelligence to streamline operations. Over the past decade, UnitedHealth's revenue has surged by 150%, a growth trajectory that analysts link to healthcare spending outpacing broader economic expansion. Consequently, the company's stock has seen a nearly 50% increase over the last year, signaling investor confidence in its turnaround.

Despite the recent upturn, a degree of market skepticism remains. Berkshire Hathaway, for instance, divested its UnitedHealth stock in the first quarter of 2026 at a loss, having acquired it at an average price of $380 in the second quarter of 2025 and selling it below $300. This move represents a rare financial setback for the conglomerate. Furthermore, the stock's valuation, with a trailing price-to-earnings ratio of 25.5 against a market capitalization of $380 billion and trailing net income of $14.9 billion, suggests that the market is banking heavily on the continued recovery of profit margins. Analysts estimate shares are trading at 23 times future earnings, implying a need for a roughly 19% increase to reclaim its previous $500 level, leaving limited tolerance for any future disappointments in the medical care ratio.

As of August 7, UnitedHealth's forward price-to-earnings ratio stands at 20.58, a notable discount compared to its trailing multiple of 25.5. This difference suggests that the market anticipates the ongoing improvement in the medical care ratio. Short interest in the stock remains modest at 2.13% of the float, indicating a limited organized bearish sentiment. However, the number of hedge funds holding the stock decreased from 145 to 130 quarter-over-quarter, a trend that appears to contradict the stock's recent rebound.

UnitedHealth's commitment to expanding healthcare access in Tennessee, coupled with its strategic financial adjustments, underscores a concerted effort to demonstrate that the challenges of 2025 were transient pricing issues, rather than fundamental structural flaws. While the company's improved second-quarter results and revised guidance offer a compelling narrative of recovery, the cautious market response, including Berkshire's strategic exit and the existing valuation metrics, indicates that the sustainability of this turnaround is still under close scrutiny.
