Remarkably, the film was brought to completion in an impressive 15-day production schedule. The cast features Joey Bader, Kaylee McGregor, Oliver Woolf, and Jack Redmayne. Tinkham was responsible for the screenplay, while Woolf, Woloson, Tinkham, and McGregor served as producers. Woloson also took on the roles of cinematographer and editor, with Noa Margalit composing the original score. The film was produced by That’s The Fish, with Haley Nicole Johnson as executive producer.

The official synopsis describes a world plunged into a mysterious conflict, where "Littermates" follows two disoriented survivors seeking refuge in a secluded English countryside cottage. Liam and Mel arrive wounded and speechless, grappling with the events that have transpired. They are taken in by Chester, an enigmatic caretaker who provides sustenance, re-educates them in speech, and institutes a meticulously controlled domestic environment. In this system, good behavior is rewarded with "nifties," tokens exchangeable for simple pleasures like karaoke, hot tub sessions, dance parties, and preferred meals. Amidst the sounds of explosions, gunfire, sirens, and karaoke, their nascent family structure begins to fray, revealing the underlying fears, loyalties, and unresolved questions that bind them.

Directors Scott Tinkham and Michael Woloson shared their inspiration, stating, "We crafted 'Littermates' to delve into the peculiar, instinctual, and often mundane dynamic between siblings.
